1. RMA request form must be completed with qty, item# / Description, chipset# / lot#, Inv# and reasons for return.
2. Please also provide date code or PCB# if they are available to you.
3. Fax or email your completed form to MTC. We will reply within 24 hours.
4. Warranty will be voided once original manufacturer's sticker is removed, covered or smudged.
5. Credit amount of modules returned for credit will be based on market price or sales price whichever is lower.
6. RMA NUMBER IS VALID FOR 14 DAYS.
